Q: Are you currently receiving Medicaid?

If YES, do nothing; you will continue to receive Medicaid.
If NO, see next question.

Q: Are you looking for Medicaid coverage?

If YES, visit benefits.ohio.gov to fill out an application online, or you can fill out an application and send it through the mail or by fax, or you can walk into your local JFS office to apply.

Q: Are you applying for aged, blind, or diabled coverage, long-term care, or waiver services?

If YES, visit odjfsbenefits.ohio.gov to fill out an application online, or you can fill out an application and send it through the mail or by fax, or you can walk into your local JFS office to apply.

Q: How do I apply if I have no access or ability to apply through the online portal or through my local agency?

You can call the Automated Health Systems at 1-800-324-8680, option 5, to apply over the phone for eligibility beginning January 1, 2014.

Q: Do you need to apply for private health insurance and not Medicaid?

Visit healthcare.gov to apply for health insurance and see what you are eligible for; whether it is a premium subsidy, tax credit, or Medicaid for coverage starting January 1, 2014 or call 1-800-318-2596.

All individuals with household ncome below 138% of the Federal Poverty Level (FPL) represented in this chart may be eligible for Medicaid unless they are covered by Medicare.

The income limits are up to 156% FPL for insured children, up to 206% FPL for uninsured children and 200% FPL for pregnant women. Therefore, pregnant women and children may quality with income at higher levels.
